UHF Report: Patient-Centered Medical Homes in New York, 2017 Update

New York State remains a leader in implementing the high-performing, cost-saving health care delivery model known as the patient-centered medical home (PCMH), in which all or most of a patient’s health care needs are coordinated through a primary care physician. UHF has been monitoring the growth of the National Commitee for Quality Assurance PCMH model since 2011; this report is an update on the most recent progress made in implementing these medical homes statewide, identifying trends and providing context on relevant changes in policy, changes in accreditation process, and various obstacles to adoption by practice type.

An accompanying appendix, included below as a separate pdf, includes 20 charts with data on PCMH growth by region (or borough) and practice type.
